چکیده

Few studies have examined adverse drug events (ADEs) in outpatient care, and identification of these events is difficult. We are using a computerized text-searching ADE monitor to assess rates of outpatient ADEs. The monitor searches notes in the outpatient electronic medical record for drug-symptom combinations that signal possible ADEs. We collected 6 months of data from outpatient sites in Boston and Indianapolis, and randomly selected a sample of possible ADEs for chart-based review. The monitor’s overall positive predictive value for ADEs was 4 %. Computerized ADE detection monitors such as this can be used in patient care to improve the routine detection of adverse drug events.

Development of a Computerized Adverse Drug Event (ADE) Monitor in the Outpatient Setting

This paper describes the collaboration of Brigham and Women’s Hospital and Regenstrief Institute to develop a computerized adverse drug event (ADE) monitor using electronic medical records from outpatient practices. We describe the steps involved in ADE monitor development and rule validation at large outpatient practices at Boston and Indianapolis. The final standard rule set adopted by both p...

Research Paper: Using Computerized Data to Identify Adverse Drug Events in Outpatients

OBJECTIVE To evaluate the use of a computer program to identify adverse drug events (ADEs) in the ambulatory setting and to evaluate the relative contribution of four computer search methods for identifying ADEs, including diagnosis codes, allergy rules, computer event monitoring rules, and text searching. DESIGN Retrospective analysis of one year of data from an electronic medical record, in...

Strategies for Detecting Adverse Drug Events among Older Persons in the Ambulatory Setting

Design: A cohort study of Medicare enrollees (n = 31,757 per month) receiving medical care from a large multispecialty group practice during a 12-month observation period (July 1, 1999 through June 30, 2000). Measurements: Possible drug-related incidents occurring in the ambulatory clinical setting were detected using signals from multiple sources. Results: During the tracking period, there wer...

Chapter 8. Computer Adverse Drug Event (ADE) Detection and Alerts

Background Adverse drug events (ADEs) occur in both inpatient and outpatient settings. Most institutions use spontaneous incident reporting to detect adverse events in general, and ADEs in particular. Spontaneous incident reporting relies exclusively on voluntary reports from nurses, pharmacists and physicians focused on direct patient care. However, spontaneous reporting is ineffective, identi...

A computerized method for identifying incidents associated with adverse drug events in outpatients

INTRODUCTION In inpatients, computer monitors have been used to improve the detection of adverse drug events (ADEs). However, similar programs have not been available in outpatients. OBJECTIVE To describe an approach for detecting incidents suggesting that an ADE may have occurred in outpatients by adapting methods from inpatient computer monitoring and developing terminology searches of elec...
